Preferred Billing Methods
You can select the default billing method to use for billable items. You can also further customize these selections for individual payers.
Out-of-Network Payment Assignments
On insurance claim forms, you may choose to have the insurance payment assigned to the patient or to your practice. This affects how patient balances are calculated and how CMS forms and claims are generated. If you assign payments to the patient, HubsteriaNotes will assume that the patient will be responsible for the full amount due. If you assign payments to the practice, HubsteriaNotes will assume the patient is only responsible for their portion of the balance. You may override this setting for individual patients. In-network payments are always assigned to the practice and patients are only responsible for their copay when billing in-network.

Paper Claims
All Paper Claims:
CMS-1500 Options:
When printing CMS-1500 information onto pre-printed red CMS-1500 forms, the printed data must line up with the pre-printed form. You can adjust the alignment below. 72 points equals one inch. Use negative values to shift up and left, and positive values to shift down and right. If you are having difficulties lining up the form, try using Adobe Reader to open the document.
All new paper claims will be generated with version 02/12 of the CMS-1500 form, as required for all claims after 4/1/2014.
Alternate Billing Address
You may optionally enter an alternate mailing address, such as a P.O. box, to be used on your bills, statements, receipts, and claims (field 33).
